Senior High


Entrance Scholarship – is automatically given to the incoming DLSMHSI Special Health Sciences Senior High students who graduated with High / Highest Honors with at least 50 total number of graduating students and more during their junior high school.

Pre-requisites:

  1. Newly admitted regular student
  2. Filipino citizen and resident of the Philippines
  3. W/ Highest Honors and w/ High Honors
  4. W/ General Weighted Average (GWA) of 95% above

Requirements:

  1. Recommendation letter from high school principal/director, certifying status as:
    1. With Highest Honors and w/ High Honors of the graduating batch with 50 total number of graduating students and more.
    2. With General Weighted Average (GWA) of 95 or higher.
  2. Photocopy of High School Report Card with General Weighted Average (GWA) / General Point Average (GPA).
  3. One (1) pc. 2×2 color picture (white background w/ nametag)

 

Academic Scholarship – is given to the DLSMHSI incoming Grade 12 students with a grade not lower than 91 in their Specialized Subjects (Foundations to Health Care 1 & 2 & General Biology) and with a grade not lower than 92 in their other subjects and w/ General Weighted Average (GWA) of at least 95% above during their Grade 11.

Pre-requisites:

  1. Regular student
  2. Filipino citizen and resident of the Philippines
  3. W/ Highest Honors and w/ High Honors
  4. With a grade not lower than 91 in Specialized Subjects (Foundations to Health Care 1 & 2 & General Biology) and with a grade not lower than 92 in other subjects.
  5. W/ General Weighted Average (GWA) of 95% above

Requirements:

  1. Letter of Intent
  2. Certificate of Moral Character
  3. Recommendation letter from the Coordinator for Academic Services duly approved by Director for Special Health Sciences High School, certifying status as:
    1. With Highest Honors and w/ High Honors of the batch
    2. With General Weighted Average (GWA) of 95 or higher.
  4. Copy of last year Grades
  5. One (1) pc. 2×2 color picture (white background w/ nametag)

 

Financial Scholarship – is given to the incoming Grade 11 and 12 students who are in need of financial assistance. Applicants are screened and evaluated based on the requirements submitted and thru interview or house visitation by the Scholarship Committee. Approved Financial Scholars are required to render at least 100-200 hours of service as Office Assistant (depends on the tuition fee discount granted) as their return service.

Pre-requisites:

  1. Regular student
  2. Filipino citizen and resident of the Philippines
  3. Has passed the background check and interview of the Senior High Scholarship Committee
  4. Minimum grade requirement of 83 per subject
  5. General Weighted Average (GWA) of 87 or higher

Requirements:

  1. Letter of Intent explaining the family’s financial situation and the urgent need for continued financial assistance.
  2. Photocopy of High School Report Card / Certification of Grades
  3. W/ a score of 160 above at the DLSMHSI Lasallian Admissions Test (LAT) (for incoming SH applicant)
  4. Certificate of Moral Character
  5. Submit the following latest certification from each parent/s / guardian/s:
    1. If the parent/s / guardian/s are employed:
      • Certificate of Employment & Compensation (including bonuses, commissions, and allowances). OFW’s must submit copy of employment contract.
      • Annual Income Tax Return or Certificate of Compensation Payment/Tax Withheld. Please indicate in your personal letter if exempted from filing an ITR or reason for non-filing then submit Affidavit of Non-Filing of Income Tax Return (ITR) instead.
      • Photocopy of pay slips for the last 3 months.
    2. If the parent/s / guardian/s are self-employed / own a business or home industry:
      • A detailed description of the nature of work or business.
      • Income & Expense Financial Statement.
      • Annual Income Tax Return. Please indicate in your personal letter if exempted from filing an ITR or reason for non-filing then submit Affidavit of Non-Filing of Income Tax Return (ITR) instead.
    3. If the parent/s / guardian/s are unemployed:
      • Certificate of Exemption from filing of Income Tax Return from the BIR or Certificate of Unemployment from the Municipal/Barangay Hall
    4. If parents are retired or were retrenched within the past three years, submit a copy of the Certificate of Retirement or Separation with the amount of retirement/separation benefits received
  1. Photocopy of billing statement (electricity, water, telephone, internet, cable, etc.) for the last three months (must submit all)
  2. One (1) pc. 2×2 color picture (white background w/ nametag)
  3. Vicinity map / sketch of the house’s location

 

Service Scholarship – is given to the DLSMHSI Grades 11 & 12 varsity players and / or choral members who are recommended by the Director for Institutional Sports and Performing Arts and Officers of The Student Affairs’ Accredited Institutional Organizations (TSAAIO).

Pre-requisites:

  1. Regular student should be at least on the 2nd year of playing as varsity or 2nd year of active membership and participation with the institutional chorale of DLSMHSI.
  2. Officers of the Institutional Student Council (ISC) (President, Vice President for Academics, Vice President for Activities, Vice President for Operations, Vice President for Finance, Vice President for Publicity and Mobilization, Executive Secretary, and SHSHS Assembly President); Editorial Board of the SHSSHS Newsletter; and Himig Sciencia (HS) (President, Vice President for Internals, Vice President for Externals, Secretary & Treasurer) and The Student Affair’s Accredited Institutional Organizations (TSAAIO)
  3. Winners in International, National, Regional, and Provincial sports and cultural competitions (recognized & accredited by the Institution)
  4. With good academic standing with grades not lower than 83.
  5. General Weighted Average (GWA) of 85 or higher
  6. Has a rating of 50% based on the Varsity/Chorale Point Scoring Checklist of the Director for Institutional Sports and Performing Arts

  3. One (1) pc. 2×2 color picture (white background w/ nametag)

 

Natatanging Gawad Lasalyanong Edukasyon – is given to the incoming DLSMHSI Special Health Sciences Senior High students who graduated from biggest science schools with High / Highest Honors and who are also in need of financial assistance. This type of scholarship is also given to the top 10 winners of National Search for Science Survivor-Wizards sponsored by the institution.

Pre-requisites:

  1. Regular student
  2. Filipino citizen and resident of the Philippines
  3. Graduate of Science schools and/or biggest public schools (whose Principals are ranked as Principal (3 or 4) in terms of the school population in CALABARZON, Metro Manila and other areas in the country or Top 10 during Final round during National Search for Sciences Survivor-Wizards
  4. With Highest Honor and with General Weighted Average (GWA) of 95 or higher.
  5. W/ a DLSMHSI Lasallian Admissions Test (CAT) score of 170 and above

Requirements:

  1. Letter of Intent explaining the family’s financial situation and the urgent need for continued financial assistance.
  2. Result of Lasallian Admissions Test (LAT with a score of 170 and above)
  3. Letter of Recommendation from high school principal/director certifying status as w/ Highest Honors
  4. Photocopy of High School Report Card (Form-138) w/ GWA
  5. Certificate of Moral Character
  6. Photocopy of billing statement (electricity, water, telephone, internet, cable, etc.) for the last three months (must submit all)
  7. Vicinity map/sketch of the house’s location
  8. One (1) pc. 2×2 color picture (white background w/ nametag)
