Admission Requirements

All potential students of DLSHSI are required to comply with the admission requirements set by the College Admissions Center and the Collegiate Admissions Committee. The Institute strictly observes the no requirements no admission policy..

General Requirements (Medicine)

  1. Completed and signed Admissions Envelope Package
  2. Original Transcript of Records (TOR)
  3. National Medical Admission Test (NMAT) Results (40% and above)
  4. Two (2) - page Accomplished Personal History Statement
  5. Photocopy of College Diploma and Certificate of Graduation in a Baccalaureate Degree
  6. Five (5) copies of 2x2 color picture
  7. General Weighted Average (GWA) / General Point Average (GPA) Certification in the Baccalaureate Degree
  8. Authenticated copy of National Statistics Office (NSO) Birth Certificate
  9. Certificate of Recommendation from previous Professor
  10. Certificate of Recommendation from an authority in the community

Additional Requirements for Foreign Applicants (Medicine)

The following documents must be duly authenticated by the Philippine Mission Abroad (PMA) in the applicant's country with diplomatic red ribbon, original signature and seal. The Embassy's notation Seen and Noted shall not be accepted.

  1. Official Transcript of Records (TOR)
  2. Notarized Affidavit of Support to cover for student's accommodation and bank certification
  3. Birth Certificate or photocopy of the data page of the applicant's passport showing the student's date and place of birth
  4. Police Clearance
  5. Payment of the $5,000 Lasallian Family Associates, Inc. Membership Fee (to be settled within the first two (2) years of stay in the College of Medicine)
