Admission Requirements

All potential students of DLSHSI are required to comply with the admission requirements set by the College Admissions Center and the Collegiate Admissions Committee. The Institute strictly observes the no requirements no admission policy..

General Requirements (Undergraduate Colleges)

  1. Completed and signed Admissions Envelope Package
  2. Authenticated Copy of the National Statistics Office (NSO) Birth Certificate
  3. HS Report Card- Form 138
  4. Allied Medical Courses Admission Test (AMCAT) Results
  5. Two (2) - page Accomplished Personal History/CAC Survey Form
  6. Photocopy of High School Diploma
  7. Certification of Moral Character from previous School Administrator
  8. Barangay Clearance/Certificate of Residency
  9. Five (5) copies of 2x2 color picture

Additional Requirements for College of Nursing Applicants

  1. General Weighted Average (GWA) of 80 or higher
  2. Belongs to upper 40% of the Graduating Batch
  3. Grades not lower than 80 in MATH, SCIENCE and LANGUAGE subjects
  4. Not more than 25 years old

Additional Requirements for Foreign Applicants

  1. Photocopy of Passport
  2. Photocopy of Alien Certificate of Registration (ACR) and Certificate of Residence for Temporary Students (CRTS)
  3. Student Visa
  4. Duly Accomplished Personal History Statement (PHS)
  5. Payment of the $500 Foreign Developmental Fee (to be settled within the first two (2) semesters of stay in the Institute)
