PRIVACY NOTICE FOR DLSMHSI PERSONNEL

 

De La Salle Medical and Health Sciences Institute (DLSMHSI) aims to comply with the Data Privacy Act of 2012 and its implementing rules and regulations and to cooperate fully with the National Privacy Commission (NPC) with its mandated role. We respect your rights to privacy as our job applicants and, eventually, as our employees; hence, this Privacy Notice.

DLSMHSI is a full-complement medical and health allied institution that provides holistic,  excellent, and premium health professions education, healthcare and research services in a nurturing God-centered environment as it pursues its commitment of Nurturing Life.

What personal data do we collect from you? When and how do we collect them?

Your personal data are collected through different manners and in various media, but mostly through written records, at various stages of your employment with us..  Your personal data can be also in the form of digital records (pictures and video) and even recordings from our CCTV and timekeeping equipment.

When you apply for a certain job or position with us, we collect the following information:  name, home address, contact numbers, date and place of birth, sex, nationality, identification numbers for SSS, Philhealth, Pag-IBIG, Tax  and  your family background.  We also gathered your educational, employment and professional histories.

When you are hired, additional information are required such as, but not limited to: (1) College Diploma and Transcript of Records; (2) PRC information details; (3) Certificates of Employment or Certificate of Good Moral Character; (4) SSS E-1 and SSS Digitized ID or Static Information (5) tax-related documents, such as your TIN ID or BIR Form 1902 (with stamp) or 2305 and 2316; (6) PhilHealth ID or  MDR or PhilHealth Member Registration Form; (7) Pag-IBIG Member’s Data Form and/or ID (8) NBI Clearance; (9) Philippine Statistics Authority (PSA) Birth Certificate (both applicant and dependents); (10) PSA Marriage Contract; (11) Psychometric exams (12) Medical Certificate/Fit-to-Work Clearance; and (13) bank account details necessary to facilitate the processing of your compensation.

During your employment with us, we also collect additional information about you, including your annual physical examination results, attendance records, group affiliation or membership and other data that may be used in the processing of your loan applications and insurance claims, in performance evaluation, and in administrative and disciplinary cases. There will also be times when we will acquire other forms of data like pictures or videos of activities you participate in, via official documentation of such activities, or through recordings from closed-circuit security television cameras installed within DLSMHSI’s premises.

You may have provided us with personal data other than yours (such as the contact details of the  person to notify in case of emergency, professional and character references and/or other similar data in nature).  In this case, you warrant that you have the permission to share those information with us.

How Do We Use Your Information?

We consider the processing of your personal data for the purposes necessary in the performance of our contractual obligations to you, for our compliance with our legal obligation, to protect your vitally important interests, including your life and health, for the performance of tasks we carry out in the public interest (e.g., public order, public safety, etc.), as an employer including a variety of administrative, research, historical, and statistical purposes. For example, we may use the information we collect for purposes such as, but not limited to:

  1. In determining your fitness to a particular job position when you are applying with us;
  2. In conducting background information check about you in relation with your employment with us;
  3. In the administration of your  payroll and other payroll-related employment benefits;
  4. In the conduct of human resource functions including performance evaluation, job promotions and even in disciplinary actions as the case may be;
  5. For submission of information to government agencies such as the Department of Labor and Employment, Department of Health, Commission on Higher Education and Department of Education and submission also to certain professional organizations for accreditation and reportorial requirements; the Social Security System, Philippine Health Insurance Corporation, Pag-IBIG, and Bureau of Internal Revenue, and other similar regulatory bodies;
  6. In complying with the corporate legal requirements such as external audits, BIR examinations and other similar regulatory activities;
  7. In providing facilities, services, security, and staff benefits to employees, as deemed necessary or in case of emergency;
  8. in the effective dissemination of information to all personnel, including the distribution of relevant newsletters and circulars and for promoting the interest of DLSMHSI as a whole;
  9. for statistical research and survey;
  10. and, other similar or related activities.

We may have to share your personal data with our affiliates and/or  related entities for employment or for secondment purposes or for organizational developments.

How do we secure your personal data? How long will we keep your data?

We shall implement reasonable and appropriate organizational, physical, and technical security measures for the protection of personal data, which we collected. The security measures shall aim to maintain the availability, integrity, and confidentiality of your personal data and are intended for the protection of personal data against any accidental or unlawful destruction, alteration, and disclosure, as well as against any other unlawful processing.

Our employees shall operate and hold personal data under strict confidentiality. Only authorized personnel have access to your personal information in our files.  They have received training on the company’s privacy and security policies to ensure confidentiality and security of your personal data.

These information will only be held as long as necessary to implement, administer, and manage your employment and to protect your rights or  to support any subsequent claims that you may have from us.

Do we share your personal data with other institutions or organizations?

As discussed above, we may have to share your information with other persons or organization in order to pursue our legitimate interests within the bound of the law and if only necessary.  Among these entities are:  the Commission on Higher Education, Department of Education, Department of Labor and Employment, the Social Security System, Philippine Health Insurance Corporation, Pag-IBIG, Bureau of Internal Revenue, Philippine Accrediting Association of Schools, Colleges and Universities and other similar bodies.

DLSMHSI may also create certain committees with a specific purpose that may also make use some of your data.  Rest assured, however, that such utilizations are within the bound of the DPA and other related laws.

What are your rights as a data subject?

As provided by the Act, you may object to the processing of your personal data, request to access your personal information, and/or have it corrected, erased, or blocked on reasonable grounds. Changes to this Privacy Policy

DLSMHSI reserves the right, acting reasonably, to modify our Privacy Policy at any time at our sole discretion, and shall publish notice of any such modification on our website.  Any modification is effective immediately upon posting.
