Registrar's Office
The Registrar’s Office serves the academic community of De La Salle Health Sciences Institute by maintaining the official records of each student and by providing appropriate data to further the educational process of the institution.

The office maintains a permanent academic record for every student and provides Transcripts of Record, Certification of Enrolment, Certification of Grades, etc. upon their request. The office is responsible for registering each student, processing of adding and dropping of subjects, distributing performance records, and recording of grades. The office keeps an up-to-date address for each student and issues of the De La Salle Health Sciences Institute ID cards.

The office maintains the permanent record for every graduate and provides Diploma, Transcript of Records, Certification of Graduation and other documents needed for licensure and employment upon their request.
