GENERAL REQUIREMENTS

  • Latest PSA Birth Certificate
  • Report of grades
    • Original Form 138 (4th year High School Report Card) (for Senior High)
    • Original Form 138 (6th year High School Report Card) (Undergraduate)
    • Original of the Transcript of Records (TOR) / Certification of Grades (Transferee / Second Course Takers, Medicine & Master of Arts in Nursing)
  • High School Diploma or Certificate of Graduation
  • Certificate of Moral Character issued by the Student Affairs and its equivalent.
  • 2pcs 2×2 color picture (white background w/ nametag)
  • Payment of the Foreign Fee of Eight Hundred Fifty Dollars ($850) during enrollment (for Foreign applicants).

ADDITIONAL REQUIREMENTS FOR UNDERGRADUATE APPLICANTS (TRANSFEREE)

  • Honorable Dismissal / Transfer Credential
  • Course / Subject Description

ADDITIONAL REQUIREMENTS FOR MEDICINE APPLICANTS

  • Original NMAT Result
  • Certificate of General Weighted Average (GWA) / General Point Average (GPA)
  • Certification of Recommendation from previous professor
  • Honorable Dismissal / Transfer Credential

ADDITIONAL REQUIREMENTS FOR MASTER OF ARTS IN NURSING

  • Certificate of recommendation from present employer (if applicable)
  • Certificate of recommendation from previous school
  • ID / Certificate of membership recognized by Philippine Nursing Association (PNA) at least 2
  • Updated PRC License
  • Study permit from present employer, if applicable

ADDITIONAL REQUIREMENTS FOR FOREIGN APPLICANTS

  • Passport
  • Alien Certificate of Registration (ACR) for permanent residents, native born, children of diplomats, or holder of any valid and Certificate of Residence for Temporary Students (CRTS)
  • Special Study Permit (SSP) for foreigners studying in the Philippines for less than a year or less than 18 years old.
  • Notarized Affidavit of Support to cover for student’s accommodation and bank certification (for Medicine applicants)
  • NBI clearance (for Medicine applicants)
  • Payment of the $850 (for senior high / undergraduate applicants) / $ 2,000 (for MAN applicants) / $ 7,000 (for Medicine applicants) Foreign fee
  • Certificate of Completion for Improving Tagalog Communication Skills from Institute’s Center for Language Training & Professional Development (CLTPD) (for Nursing and Medicine applicants).
