Online Application for Senior High Applicants


  1. Fill out the Online Application Form and follow the instructions given.

    Please check the announcement to see the available programs before applying

  2. Pay the non-refundable / non-transferable Php 500.00 (for Filipino applicants) / $125.00 (for Foreign
    applicants) Lasallian Admission Test (LAT) fee through Dragonpay (see
    the link for the bank details and video for the payment procedures).
  3. After you receive your Official Receipt from our cashier, log-in to your applicant’s portal & choose your preferred Entrance Examination schedule.

    Note: Online Exam is available for applicants from outside Cavite only. Please email first kmgabiana@dlsmhsi.edu.ph (for incoming Senior High) / pfcampana@dlsmhsi.edu.ph (for incoming Undergraduate) requesting for online exam together with your full name & address (1st come 1st served since limited slots are available & subject for approval).

  4. Read and watch the written and video guidelines provided and other details for your entrance examination.

    • Take the “Practice examination” as soon as possible (during your vacant time) so that you could be familiarized with the general aspects of the exam & if ever you encounter problems opening the software, the TRAST may assist and solve the problem before your actual exam.
    • Take the Online LAT as scheduled. The applicant should be online and log-in on their account at least 30 minutes before the examination (please take note that the applicants are expected that they already read the Guidelines on taking the DLSMHSI Online Entrance Examination and already took the Practice Exam).
    • A rescheduling fee of Php 300.00 shall be pay if the applicant fails to attend or wish to reschedule their entrance examination.
    • The Online Lasallian Admissions Test (LAT) result shall be released through your applicant’s portal after finishing the entrance examination.

Screening and Evaluation

  1. Once the applicant has decided on a course of choice, pay the Php600.00 Admissions Processing fee through Dragonpay (see the link for the bank details and video for the payment procedures).
  2. Log-in to your applicant’s portal and choose your preferred interview schedule.
  3. Read the written guidelines provided and other details for your interview.

    • A rescheduling fee of Php 300.00 shall be pay if the applicant fails to attend or wish to reschedule their interview schedule.
  4. Check your applicant’s portal at least 1-2 days after your interview or contact TRASTS for the final result of the evaluation.


  1. Secure Acceptance Letter from TRAST through your portal with the link for the complete instructions on the next step should the applicant pass all the criteria for screening and evaluation.
  2. Pay thePhp 10,000 reservation fee through Dragonpay (see the link for the bank details and video for the payment procedures) to confirm your slot. The reservation fee is non- refundable and non-transferable but should the applicant enroll in this institution, the aforementioned amount shall be deducted from his / her total tuition fee. Applicants who do not comply with all the requirements regardless of payment shall have their reservation fees forfeited.Deadline for reservation of slot is one month upon receiving the Acceptance Letter. Failure to comply with the aforementioned policy would mean forfeiture of slot.
  3. Email us the following requirements at TRASTSrequirements@dlsmhsi.edu.ph (forms are available at http://www.dlshsi.edu.ph/academics/admission/downloadable-forms/):

    • Proof of payment (validated deposit slip)
    • Completed & signed Acknowledgement Letter (for Filipino /
      Foreign Applicants)
    • Completed Applicant Personal History Statement
    • Completed & signed Declaration of Agreement
    • Completed & signed Special Needs Assessment Form
    • Completed & signed Declaration of Citizenship

    Specifying the following:

  4. Log-in to your applicant’s portal and choose your medical examination and uniform measurement schedule.
  5. Submit the following original requirements thru personal visit (every Monday-Saturday 8:00 am – 4:00 pm) or thru snail mail/courier with the following information for you to receive your Enrollment Permit and Enrollment procedure in your applicant’s portal.

    1. Original PSA Birth Certificate.
    2. Original Form 138 (4th year High School Report Card).
    3. Photocopy of Certificate of Moving-up.
    4. Certificate of Moral Character issued by the Student Affairs and its

    Additional Requirements for Foreign applicants

    1. Photocopy of Passport (for foreign applicants).
    2. Photocopy of Alien Certificate of Registration (ACR) for permanent
      residents, native born, children of diplomats, or holder of any
      valid visa and Certificate of Residence for Temporary Students
      (CRTS) (for foreign applicants).
    3. Special Study Permit (SSP) for foreigners studying in the
      Philippines for less than a year or less than 18 years old (for
      foreign applicants).

    For applicants who cannot submit the said original documents on or before enrollment, you can submit the Original Notarized Provisional Enrollment Form for you to enroll temporarily.   Once the lacking requirements are available before the specified date on your Notarized Provisional Enrollment Form , you may submit them to the TRAST through personal visit (every Monday-Saturday 8:00 am – 4:00 pm) or through snail mail/courier at the given address for us to update your enrollment status.
