Online Application for Master of Arts in Nursing Applicant

Screening and Evaluation

  1. Fill out the Online Application Form and follow the instructions given. (for, DLSMHSI student, you need to log-in to your Student’s Portal. Click “Enrollment” then input the school year you are applying for then choose “Master of Arts in Nursing” and finally click “apply for 2nd course”).
  2. Pay the Php2,000.00 (for Filipino applicant) / $250.00 (for Foreign applicant) Admissions Processing fee through Dragonpay (see the link for the bank details and video for the payment procedures).
  3. Submit the following requirements and information together with the proof of payment (validated deposit slip) to The Registrar – Admissions, Scholarships, and Testing Section (TRASTS) thru email to postgradadmission@dlshsi.edu.ph specifying the following:
    SUBJECT: Master of Arts in Nursing Applicant

    General Requirements:

    • Transcript of Records (TOR)
    • Color picture
    • College diploma
    • Certificate of recommendation from present employer, if applicable OR Certificate of recommendation from previous school OR Study permit from present employer, if applicable
    • Photocopy of Philippine Nursing Association (PNA) ID, PRC License and (1) certificate of membership in any recognized nursing organization
    • Study permit from present employer, if applicable
    • Curriculum Vitae
  4. Once you receive a confirmation email from TRAST, log-in to your applicant’s portal & choose your preferred interview schedule.
  5. Read the written guidelines provided and other details for your interview.
    • A rescheduling fee of Php 300.00 shall be pay if the applicant fails to attend or wish to reschedule their interview schedule.
  6. Check your applicant’s portal at least one (1) week after your interview or contact TRASTS for the final result of the evaluation.


  1. Secure Acceptance Letter from TRAST through your portal with the link for the complete instructions on the next step should the applicant pass all the criteria for screening and evaluation.
  2. Pay the Php 10,000.00 reservation fee through Dragonpay (see the link for the bank details and video for the payment procedures) to confirm your slot. The reservation fee is non- refundable and non-transferable but should the applicant enroll in this institution, the aforementioned amount shall be deducted from his / her total tuition fee. Applicants who do not comply with all the requirements regardless of payment shall have their reservation fees forfeited. Failure to comply with the aforementioned policy would mean forfeiture of slot.
  3. Email us the following requirements at TRASTSrequirements@dlsmhsi.edu.ph & please cc jplachica@dlshsi.edu.ph specifying the following (forms are available at http://www.dlshsi.edu.ph/academics/admission/downloadable-forms/):
    SUBJECT: {Program Applying} Reservation of Slot – Master of Arts in Nursing
    • proof of payment (validated deposit slip)
    • Completed & signed Acknowledgement Letter
    • Completed & signed Applicant Personal History Statement
    • Completed & signed Declaration of Agreement
    • Completed & signed Special Needs Assessment Form
    • Completed & signed Declaration of Citizenship
  4. Submit the following original requirements thru personal visit (every Monday-Saturday 8:00 am – 4:00 pm) or thru snail mail/courier with the following information for you to receive your Enrollment Permit and Enrollment procedure in your applicant’s portal.

    • Transfer Credential / Honorable Dismissal from previous school
    • Original PSA Birth Certificate

    Additional Requirements for Foreign applicants

    • Photocopy of Passport (for foreign applicants).
    • Photocopy of Alien Certificate of Registration (ACR) for permanent residents, native born, children of diplomats, or holder of any valid visa and Certificate of Residence for Temporary Students (CRTS) (for foreign applicants).
    • Special Study Permit (SSP) for foreigners studying in the Philippines for less than a year or less than 18 years old (for foreign applicants).

    For applicants who cannot submit the said original documents on or before enrollment, you can submit the Original Notarized Provisional Enrollment Form & Sworn Statement for you to enroll temporarily.

    Once the lacking requirements are available before the specified date on your Notarized Provisional Enrollment Form , you may submit them to the TRAST through personal visit (every Monday-Saturday 8:00 am – 4:00 pm) or through snail mail/courier at the given address for us to update your enrollment status.
