Guidelines for library patrons

  1. The Library shall strictly implement an appointment-based access to the library spaces, resources and technology. All library visitors must schedule their appointment online (https://forms.gle/poW7rW2hWhjmvGrP7) at least a day prior to the date of visit. All visitors shall only be allowed to use the library spaces and technology for a maximum of 4 hours, as stipulated in the “Guidelines for Students’ Short–term Academic transaction and Stay at DLSMHSI Facilities of the Student Affairs”. Confirmation/Cancellation of appointment shall be sent via email. No undergraduate and senior high school students shall be accepted unless the Institution allows their access and use of facilities.
  2. Before coming to the Library, all visitors must proceed to the Student Affairs to secure a numbered Gate Pass and Covid-19 Screening Pass or Clearance Slip from the Student Fellow and Generative Assistants. The Gate Pass and clearance slip shall be presented to the guard and staff on duty at the library.
  3. At the library section entrance, all visitors shall be asked to fill-up the daily transaction form provided at the OPAC station to monitor the duration of stay for each visitor following the intended transaction.
  4. Wearing of protective mask and shield is required. Frequent hand washing and regular use of disinfectant solution shall be observed.
