DLSMHSI SCHOLARSHIP PROGRAM FOR INCOMING/CURRENT UNDERGRADUATE

Reminders:

  • Please check the timetable/application period. Even if you are qualified for the scholarship you are applying for, you cannot avail of the tuition fee discount if you do not submit your application and the requirement requirements before the set deadline for application. DLSMHSI is very strict in following the timetable.
  • If you want to apply for multiple types of scholarship, then you need to fill out application and upload separate requirements for each of the types of scholarship.
  • Please read the Scholarship Policies and Guidelines for the updated additional requirements set per college.
  1. Entrance Scholarship – is given to the incoming DLSMHSI undergraduate students who graduated with High / Highest Honors with at least 50 total number of graduating students and more during their senior high school.
    Pre-requisites:

    1. Newly admitted regular student
    2. Filipino citizen and resident of the Philippines
    3. W/ Highest Honors and w/ High Honors
    4. Belongs to:
      1. Top twenty (20) examinees of the DLSMHSI Entrance Scholarship Qualifying Exam – for incoming Nursing and Medical Laboratory Science students
      2. Top ten (10) examinees of the DLSMHSI Entrance Scholarship Qualifying Exam – for other undergraduate programs offered.

    Application Procedures:

    • Fill out the Scholarship Application Form for Entrance Scholarship-UG.
    • Upload and fill-out the following requirements on the assigned google drive (Please rename the files to be uploaded as (Last Name, First Name, Middle Name & Temporary/Student ID number) & in pdf format).
      Requirements:

      1. Completed and signed Entrance Scholarship Recommendation Form by the School Principal / Director certifying status as W/ Highest Honors and w/ High Honors of the graduating batch with 50 total number of graduating students and more.
      2. High School Report Card with General Weighted Average (GWA) / General Point Average (GPA).

  2. Academic Scholarship – is given to the DLSMHSI undergraduate students with a grade not lower than 85 in their subjects and w/ General Weighted Average (GWA) of at least 92% above during their stay in the institution / Has met the grade requirement set by College
    Pre-requisites:

    1. Regular student
    2. Filipino citizen and resident of the Philippines
    3. With at least a grade not lower than 85 in all subjects
    4. General Weighted Average (GWA) qualification of 92% or higher / Has met the grade requirement set by College

    Application Procedures:

      Requirements:

  3. Financial Scholarship – is given to the incoming DLSMHSI undergraduate students who are in need of financial assistance. Applicants are screened and evaluated based on the requirements submitted and thru interview or house visitation by the Scholarship Committee. Approved Financial Scholars are required to render at least 50-100 hours of service as Office Assistant (depends on the tuition fee discount granted) as their return service.
    Pre-requisites:

    1. Regular student
    2. Filipino citizen and resident of the Philippines
    3. Has passed the background check and interview of the Senior High Scholarship Committee
    4. Minimum grade requirement of 83 per subject (depends on the existing College policy)
    5. General Weighted Average (GWA) of 87 or higher (depends on the existing College policy)

    Application Procedures:

      Requirements:

      1. Letter of Intent explaining the family’s financial situation and the urgent need for continued financial assistance.
      2. High School Report Card / Certification of Grades
      3. Certificate of Moral Character
      4. Preferably w/ a score of 160 above at the DLSMHSI College Admissions Test (CAT)
      5. Certificate of Moral Character
      6. Billing statement (electricity, water, telephone, internet, cable, etc.) for the last three months (must submit all)
      7. Vicinity map / sketch of the house’s location

      Requirements (for renewal):

      1. Letter of Intent explaining the family’s financial situation and the urgent need for continued financial assistance.
      2. Certificate of Moral Character (you can request your Good Moral at sds-tsa@dlshsi.edu.ph)
      3. Duty Hours completed based on the number of hours need to finish
      4. Completed and Signed Performance Evaluation Form by the Head/Dean/Director of the office assigned.

  4. Service Scholarship – is given to the DLSMHSI Officers of Institutional Student Council.
    Application Procedures:

    1. Fill out the Scholarship Application Form for Service Scholarship-UG.
    2. Upload and fill-out the following requirements on the assigned google drive (Please rename the files to be uploaded as (Last Name, First Name, Middle Name & Temporary/Student ID number) & in pdf format).
      Requirements:

  5. Gawad Lasalyanong Edukasyon – is given to the incoming DLSMHSI undergraduate students who graduated from biggest science schools with High / Highest Honors and who are also in need of financial assistance.
    Pre-requisites:

    1. Regular student
    2. Filipino citizen and resident of the Philippines
    3. Graduate of Science schools and/or biggest public schools (whose Principals are ranked as Principal (3 or 4) in terms of the school population in CALABARZON, Metro Manila and other areas in the country
    4. With Highest Honor and with General Weighted Average (GWA) of 95 or higher.
    5. Preferably w/ a DLSMHSI College Admissions Test (CAT) score of 170 and above

    Application Procedures:

    • Fill out the Scholarship Application Form for GLE Scholarship-UG.
    • Upload and fill-out the following requirements on the assigned google drive (Please rename the files to be uploaded as (Last Name, First Name, Middle Name & Temporary/Student ID number) & in pdf format).
      Requirements:

      1. Letter of Intent explaining the family’s financial situation and the urgent need for continued financial assistance.
      2. Result of College Admissions Test (CAT with a score of 170 and above)
      3. Letter of Recommendation from high school principal/director certifying status as w/ Highest Honors
      4. High School Report Card (Form-138) w/ GWA
      5. Certificate of Moral Character
      6. Photocopy of billing statement (electricity, water, telephone, internet, cable, etc.) for the last three months (must submit all)
      7. Vicinity map/sketch of the house’s location

      Requirements (for renewal):

      1. Letter of Intent
      2. Certificate of Moral Character (you can request your Good Moral at sds-tsa@dlshsi.edu.ph)
      3. Copy of grades (screenshot from institution’s portal will do)

    • Tuition Fee Discount:
      Full tuition including miscellaneous, special assessment and other fees including book and uniform allowances, physical and other medical examinations required by the Institution.

  6. DLSMHSI Scholarship to Employment Program
    Nature of the Scholarship and Employment Opportunities

    • This shall be offered to qualified graduating students who may be interested to avail of the full scholarship opportunities provided by the DLSMHSI, first and second semesters, then 2-year employment opportunity at the De La Salle University Medical Center immediately after passing the licensure examinations.
    • This also covers full subsidy on the licensure examination reviews (review fees,application fees, board and lodging, and all other related expenses).
    • This program shall initially run for 3 school years, then to be assessed after, depending on the turnout of applicants at the DLSUMC and its manpower needs.

    NOTE: Dependents of DLSMHSI employees who have started to avail of the Enhanced Brother President Scholarship Program may be allowed to apply for the said scholarship during their fourth year subject to the screening process and policies and guidelines set for this scholarship program.

    THRUST: From Scholarship to Employment
    Pre-requisites

    1. Filipino citizen residing in the Philippines;
    2. 4th year / incoming 4th year student of the following programs:
      1. BS Nursing
      2. BS Pharmacy
      3. BS Medical Laboratory Science
      4. BS Physical Therapy / Occupational Therapy
      5. BS Radiologic Technology
    3. Willing to sign the Contract for the DLSMHSI Scholarship to Employment Scholarship program (see the Memorandum of Agreement)

    Application Procedures:

    • Fill out the Scholarship Application Form for SEP Scholarship-UG.
    • Upload and fill-out the following requirements on the assigned google drive (Please rename the files to be uploaded as (Last Name, First Name, Middle Name & Temporary/Student ID number) & in pdf format).
      Requirements

      1. Letter of Intent addressed to the Vice Chancellor for Academics
      2. Curriculum Vitae
      3. Certificate of Moral Character (you can request your Good Moral at sds-tsa@dlshsi.edu.ph)
      4. Recommendation Letter from the College Deans or their authorized representative/s
      5. Copy of the Certificate of Grades from Year Level 1 to Year level III issued by The Registrar
      6. Parent/s’ Consent Form

      Screening Process

      1. All interested applicants shall submit to The Registrar – Admissions, Scholarship and Testing Section (TRAST) the Application Letter addressed to the Vice Chancellor for Academics together with the Curriculum Vitae, Recommendation Letter from the College Deans or their authorized representative/s, Certificate of Moral Character issued by The Student Affairs, Copy of the Certificate of Grades from Year Level 1 to Year level 111 issued by The Registrar, and Parent/s’ Consent Form.
      2. The documents submitted shall be forwarded by the TRAST to the Scholarship Committee of the college concerned.
      3. The submission of the final recommendation by the Scholarship Committee to the Office of the Vice Chancellor for Academics for final approval shall be done immediately after the evaluation.
      4. The Office of the Vice Chancellor for Academics through TRAST shall release the results and communicate with the successful applicants.

      Scholarship Privileges

      • Full tuition, miscellaneous and other fees (4thyear-first and second semesters)
      • Full subsidy on licensure exam reviews which includes board and lodging, application fee, transportation (exam site), review materials, and other related expenses

      NOTE: The scholars may choose their own preferred review centers so long they are accredited and recognized by their respective colleges.

      Employment Opportunities

      • 2-year employment contract at the DLSUMC (entitled to receive the prevailing salaries and benefits and other package given by the hospital)

      NOTE: A very impactful and well-written employment certification and endorsement in case overseas employment is considered after fulfilling the return service agreement shall be issued by the DLSUMC and HRM Department.

  7. Externally Funded Scholarships
    • Requirements and policies for this type of scholarship are set by the sponsoring organizations and are evaluated by the offices concerned but the applicants are required to file their applications at The Registrar – Admissions, Scholarships and Testing Section.
  8. Brother President Scholarship Program (BPSP) / Enhanced Brother President Scholarship Program (EBPSP) / Employees’ Dependents (EDSP) / Grand Father Clause (GFCSP) Scholarship Program
    • Are scholarship benefits given to the children of DLSMHSI regular employees who wish to enroll in DLSMHSI and DLSU-D.

    Pre-requisites:

    1. Regular employee of De La Salle Medical and Health Sciences Institute
      Requirements:

      1. Application Form
      2. Original copy of the Certificate of Employment of the applicant faculty or staff, issued by Human Resource Management with certification that the applicant has been granted a permanent status
      3. Clear copy of the birth Certificate of the dependent
      4. Adoption papers if dependent is legally adopted
      5. Prospectus of the course enrolled in
      6. Certificate of Admission from the school or course enrolled in
      7. One (1) pc. 2×2 color picture (white background w/ nametag)

Application Guidelines for Undergraduate

  • Entrance Scholarship is automatic but the scholars are required to file an application form. They are also required to fill-out the Scholarship Application Form and attach the proof / certification of honor and submit it to The Registrar – Admissions, Scholarships and Testing Section (TRAST) upon filing an application for admission
  • Gawad Lasalyanong Edukasyon Scholarship is automatically given to qualified applicants and is renewed every year. However, all scholars are required to fill-out the Scholarship Application Form, attached all the requirements stipulated in the form, and submit it to the TRAST upon filing and application for admission. They are also required to submit the copy of their grades every semester.
  • Academic, Financial, Service, BSP, EBPSP, EDSP / GFCSP, and GLE scholar applicants need to file their Scholarship Application Form and complete all the requirements stipulated in the form on or before the set deadline of application.

Significant Details / Information Relatable to Scholarships

  • All scholar-applicants are required to submit their letter of intent addressed to their Collegiate Scholarship Committee Chairs, fill-out the Scholarship Application Form and submit the requirements stipulated on or before the set deadline by The Registrar – Admissions, Scholarships and Testing Section (TRAST). Old grantees may only submit additional information for updating purposes.
  • All applications shall be pre-screened by The Registrar – Admissions, Scholarships and Testing Section (TRAST) before forwarding the documents to the colleges concerned through the Collegiate Scholarship Committee chairs and College Deans / Directors. The college through the Scholarship Committee then evaluates the documents (the Scholarship Committee chair may opt to call for an interview with the applicant as the application process continues) and submits prioritized list of candidates for scholarships to the TRAST. The types of scholarships to be awarded depend on what is/are being applied for by the scholar applicants. However, TRAST through the Scholarship Committee may award a different type of scholarship if the applicants have been disqualified from their desired type and that available slots from other types of scholarships have been identified and the applicants themselves have satisfied all of the requirements. The TRAST may also recommend to the President’s Scholarship Fund if slots are still available and the applicants satisfy same requirements.
  • All prospective scholars under the fund assistantship of the Brother President through individual sponsorships shall also be pre-screened by The Registrar – Admissions, Scholarships and Testing Section (TRAST). Upon recommendation of the TRAST (submission of the list of qualified applicants), the Brother President shall now choose the applicants who shall be granted scholarships.
  • All scholar-applicants must wait for the release of the final evaluation report by The Registrar – Admissions, Scholarships and Testing Section (TRAST) and the Vice Chancellor for Academics, the Brother President in the case of the BPSG scholar-applicants and the Vice Chancellor for Administration and Finance in the case of the BPSP, EBPSP, EDSP / GFCSP, and SLMEB scholar-applicants. Likewise, they are advised to wait for further announcements (through the TRAST Bulletin of Information, DLSMHSI website, and other correspondences). The final list of scholars shall be posted in the DLSMHSI TRAST website. The scholars are enjoined to visit the office for the confirmation of their status.
  • All undergraduate scholars are enjoined to renew/re-apply for their scholarships every semester including the EBPSP, EDSP / GFCSP and SLMEB.
  • All scholars must fill-out and submit the Memorandum of Agreement (MOA) at the beginning of every school year. The MOA contains all the policies and guidelines, conditions of scholarships, the provisions for the mandatory return of service, etc.
  • The Registrar – Admissions, Scholarships and Testing Section (TRAST) prepares the consolidated list of approved scholars and their completed and signed Memorandum of Agreement (MOA) to be submitted to the Vice Chancellor for Academics and Vice Chancellor for Administration and Finance for final approval.

Grounds for Denial of Scholarship Application

  • Failure to comply with scholarship policies
  • Any violations against the Memorandum of Agreement (MOA)
  • Deficiency in requirements
  • Falsification of documents
  • Commission of major offense from The Student Affairs of DLSMHSI, college of origin or community
  • Unavailability of slots and/or budget for the scholarship applied for
