Submit the following requirements to the Office of the Medical Director:

  1. Accomplished Application Form (2 copies)

  3. Passport size photo (2 copies)
  4. Letter of Application addressed to the Department Chairman thru the
    Section Chief
  5. Letter of Recommendation from three (3) persons
  6. Original copy (for authentication) and photocopy of the following:

    1. Diploma:
      • College of Medicine
      • Certificate of Internship
      • Certificate of Residency Training
      • Specialty Board Certification
    2. Diploma: College of Medicine Certificate of Internship
      Certificate of Residency Training Specialty Board Certification
    3. Transcript of Records (Medicine)
    4. Board Rating
    5. PRC Certificate
    6. PRC License
    7. Narcotic License Number (S2)
  7. Drug test result copy
  8. Tax Identification Number (TIN) ID
  9. Copy of Certificate of Registration (BIR Form 2303)
  10. NBI or Police Clearance
  11. Professional Tax Receipt (PTR) latest
  12. Community Tax Receipt (cedula)
  13. Psychometric Examination Result (referral slip c/o Medical Director’s
    Office)
  14. Physical Examination Result (referral slip c/o Medical Director’s
    Office)
