APPLICATION FOR GRADUATION
The Application for graduation is applied to graduating students of any course/program to undergo an exit interview through portal (online). Likewise, they shall have accomplished the Pre-Employment forms. This is conducted on the latter period of academic year before the graduation. These are one of the requirements for a graduating student to be qualified to apply for graduation. The Alumni Relations shall ascertain that all documents are completely fill-out by the graduating students before signing their graduation clearance.
ONLINE FEEDBACK SURVEY
- Log on to the Alumni Relations website at alumni.dlshsi.edu.ph
- Log in using the code from the printouts given during application for graduation.

The Alumni Relations shall facilitate to graduating students in the application of the Social Security (SSS) and PhilHealth as part of the requirements needed in future career job placement.
JOB FAIR SUPPORT
This refers to job hunting activity gathered in one place which is offered to Alumni of the De La Salle Medical and Health Sciences Institute for job placement in the market.
REVIEW PROGRAM ASSISTANCE
This offers to alumni graduates of DLSMHSI who have not passed the professional licensure examination.
ALUMNI IN- RESIDENCE SCHOLARSHIP GRANT
The Scholarship Grant is offered to a graduating student of DLSMHSI who cannot afford to continue to financially support one’s education. This grant is subject to availability of funds.
LASALLIAN FORMATION SUPPORT
This refers to provide support to the alumni on how to maintain and lived the core values of St. John Baptist De La Salle through providing a Lasallian formation education.
GRAND ALUMNI HOMECOMING/REUNION SUPPORT
The Alumni Relations fosters strong connection with our alumni through support of their activities.
Lasalyanong Pagmamahal (Lasallian love support)
This demonstrates of a lasallian love to the alumni who seek help and support as well as extend this help to the external alumni especially to less fortunate individuals confined at the DLSUMC charity ward.
