  4. Letter of Application addressed to:
    • Miguel E. Barbosa, MD, FPCEM
    • Chair, Department of Emergency Medicine
  5. Letter of Recommendation from three (3) persons
  6. Original copy (for authentication) and photocopy of the following:
    1. Medicine Diploma
    2. Certificate of Internship
    3. Certificate of previous residency (if any)
    4. Transcript of Records (Medicine)
    5. PRC Board Rating
    6. PRC Certificate
    7. PRC Identification Card
    8. Narcotic License Number (S2)
  7. NBI or Police Clearance
  8. SSS E1 and Number
  9. Latest Professional Tax Receipt (PTR)
  10. Tax Identification Number (TIN)
  11. Community Tax Certificate (cedula)
  12. Psychometric evaluation result
  13. Physical examination result
