SCHOLARSHIP APPLICATION PROCEDURES FOR DLSMHSI EMPLOYEE

Brother President Scholarship Program (BPSP) / Enhanced Brother President Scholarship Program (EBPSP) / Employees’ Dependents (EDSP) / Grand Father Clause (GFCSP) Scholarship Program

  • Are scholarship benefits given to the children of DLSMHSI regular employees who wish to enroll in DLSMHSI and DLSU-D.

Pre-requisites:

  • Regular employee of De La Salle Medical and Health Sciences Institute

Application Procedures (for new applicants):

  1. Fill out the Scholarship Application Form for DLSMHSI Employee (New).
  2. Download and fill-out the following fillable Scholarship Forms:
    1. Scholarship Application Form (to be fill-out by the student)
    2. Employee Scholarship Application Form
      1. Employee’s Dependents (EDSP) Scholarship Program / Grand Father Clause Scholarship Program (GFCSP)
      2. Enhanced Brother President Scholarship Program (EBPSP)
      3. Romeo P. Ariniego, MD Scholarship Grant (RPAMDSG)
      4. St. La Salle Medical Education Benefit (SLSMEB)
    3. Memorandum of Agreement (MOA)
      1. Employee’s Dependents (EDSP) Scholarship Program / Grand Father Clause Scholarship Program (GFCSP)
      2. Enhanced Brother President Scholarship Program (EBPSP)
      3. Romeo P. Ariniego, MD Scholarship Grant (RPAMDSG)

    4. Authorization Letter
      1. Employee’s Dependents (EDSP) Scholarship Program / Grand Father Clause Scholarship Program (GFCSP)

      4. St. La Salle Medical Education Benefit (SLSMEB)
  3. Upload following requirements together with the completed & signed Scholarship Forms on the assigned google drive (Please rename the files to be uploaded as (Last Name, First Name, Middle Name & Temporary/Student ID number) & in pdf format).

    Requirements:

    1. PSA Birth Certificate of employee & student
    2. Adoption papers if dependent is legally adopted
    3. Certificate of admission in the school or course enrolled in
    4. Prospectus of the course enrolled in
    5. Employment Certificate
  4. Please read the reminder shown on the last part of google form per type of scholarship.

 
For renewal applicants
Application Procedures (for new applicants):

  1. Fill out the Scholarship Application Form for DLSMHSI Employee (Renew).
  2. Download and fill-out the following fillable Scholarship Forms:
    1. Scholarship Application Form (to be fill-out by the student)
    2. Employee Scholarship Renewal Application Form
      1. Employee’s Dependents (EDSP) Scholarship Program / Grand Father Clause Scholarship Program (GFCSP)

    1. Registrar-certified true copy of grades earned during the previous semester
    2. Proof of re-enrolment in the subject/s failed, if any
    3. Proof of payment for the re-enrolled subject/s failed, if any
  4. Please read the reminder shown on the last part of google form per type of scholarship.
